About the Role:

As an Ambulatory Care Clinical Coordinator (pharmacist), you will play a crucial role in overseeing and enhancing our pharmacy program. You will have the opportunity to develop and implement quality improvement initiatives, educate clinicians and pharmacists, analyze pharmacy data, and collaborate with a multidisciplinary team to deliver safe, effective, and cost-efficient patient care. This is a fully remote position, but you must reside in one of the following states: Oregon, Washington, Utah, Idaho, Arizona, Nevada, Texas, Montana, or Wisconsin.

What You'll Do:

Develop and conduct quality improvement programs related to the pharmacy program.
Create and implement clinical educational programs to enhance drug utilization and quality.
Develop educational initiatives to improve prescribing patterns for quality and cost-effectiveness.
Analyze pharmacy claim data and generate reports to support program objectives.
Conduct retrospective drug use reviews.
Review medication prior authorization requests and appeals.
Refine policies and procedures related to pharmacy functions (medication therapy management, drug utilization review, prior authorization).
Perform new drug reviews and develop formulary recommendations.
Assess and respond to federal and state regulatory requirements/audits.
Consult with clinicians and pharmacists to resolve pharmacy benefit issues.
Precept pharmacy students and residents.
Evaluate drug therapy regimens for care coordination patients and assist with treatment plans.
Provide medication therapy management (MTM) services.
Utilize technology platforms for documentation and MTM objectives.
Monitor MTM objectives and high-risk population outcomes.
Perform medication reviews and other tasks to meet plan quality metrics.
What You'll Need:

Graduate of an accredited pharmacy program.
Current (or eligible) unrestricted pharmacist license in the state of Oregon.
Advanced pharmacy training (PharmD, residency, fellowship, board certification, or master's degree).
Two years of experience as a clinical pharmacist in pharmaceutical care management (ambulatory care or other clinical setting, including residency experience).
Preferred Qualifications:

Previous experience in managed care and project management.
PGY1 residency training.
Required Knowledge, Skills, and Abilities:

Knowledge of managed care principles, pharmacy benefit management, reimbursement, and drug utilization.
Comprehensive clinical pharmaceutical knowledge.
Ability to evaluate clinical literature and apply evidence-based medicine principles.
Ability to design and review pharmacy claims analysis reports.
Self-motivated and able to work independently.
Excellent organizational, project management, and time-management skills.
Excellent written and verbal communication skills.
Excellent customer service skills.
Ability to work effectively with diverse individuals and groups.
Ability to manage multiple tasks, negotiate, problem-solve, and build consensus.
Proficiency in word processing, spreadsheets, and databases.
